Please complete this form to confirm your participation in the Lauda Musicam concert at St. Bartholomew’s Episcopal Church on Sunday, May 5, 2024. There will be a dress rehearsal at St. Bart’s 11 am–4 pm (perhaps shorter) on Saturday, May 4.

Please indicate which line you play as well as which instrument you will play on that line. In the case of “El Fuego,” give a brief description if you are playing multiple lines.

If you completed a similar form for Brasstown, you do not need to resubmit your part information UNLESS IT HAS CHANGED. Changes I requested for Brasstown do not have to carry into the St. Bart’s concert, but you may play your new line if you wish.
